A passport photo or similar (head and shoulders with a light background) is required with your application. Passport photos are available at Post Offices and several commercial sources (i.e., Walgreen’s, CVS, etc.). Quality digital photos meeting the stated criteria may be sent via email.

By submitting this application, you certify that all information given is true, and that you authorize the CIRC/ICC permission to conduct a national records check, including a criminal background check. Providing incomplete or fraudulent information will be grounds for membership denial and/or dismissal.

- Personal information is not published, sold or given to other entities. Personal information is held in strict confidentiality, and is only used for confirmation of your acceptance and endorsement. In the event you are not approved for membership, your application fee will be refunded in full.

- CIRC/ICC members minister to all persons in need regardless of their faith or religious affiliation, including those who have no faith.

Please include $75.00 to cover the processing of your application. If your application is approved you will receive: A Membership Certificate, official photo ID, uniform patch and a CIRC/ICC Chaplain’s Commission (if you are not a legally ordained or licensed Christian minister).

Members pay an annual renewal fee of $25.00 to offset the cost of a new I.D. and updated criminal background check every three years.
